Vaccine Updates Needed For Kindergartners, 7th Graders, And Seniors

Families of kindergartners, 7th Graders, and PHS seniors are reminded that vaccinations for your students are due. Considering the uniqueness of our year, the deadline is September 30.

For kindergartners
We must have records for every student, including those who will attend virtually.
Records should include: Proof of infant vaccinations AND recently received: DTap (Tetanus, Diphtheria and acellular Pertussis), IPV (Polio), MMR (Measles, Mumps, Rubella), and Varicella (Chicken Pox) vaccines.

Ohio law permits exemptions for students who have had natural immunity due to previous illness, medical contraindications, and for reasons of conscience or religion.

Exemption forms may be requested at any time and are also due before September 30.

Records can be sent or dropped off at the Furry main office or emailed to nurse Melissa Koch at mkoch@perkinsschools.org.

For 7th Graders
We must have records for every student, including those who will attend virtually.
Records should include a booster dose of Tdap (Tetanus, Diphtheria and acellular Pertussis) and their first dose of MCV4 (Meningococcal serogroup A, C, W, and Y).

These are the minimum required for school however, there are additional optional vaccines that may be discussed with your child's healthcare provider.

Ohio law permits exemptions for students who have had natural immunity due to previous illness, medical contraindications, and for reasons of conscience or religion.

Exemption forms may be requested at any time and are also due before September 30.

Records can be sent/dropped off at the Briar main office or emailed to nurse Heather Zess at hzess@perkinsschools.org.

For PHS Seniors
We must have records for every student, including those who will attend virtually and those attending EHOVE or Firelands-BGSU.
Records should include a dose of MCV4 (Meningococcal serogroup A, C, W, and Y) sometime after age 16.

This is the minimum required for school however, there are additional optional vaccines that may be discussed with your child's healthcare provider.

Ohio law permits exemptions for students who have had natural immunity due to previous illness, medical contraindications, and for reasons of conscience or religion. Exemption forms may be requested at any time and are also due before September 30.

Records can be sent/dropped off at the main office of PHS or emailed to nurse Lisa Miller at lisa.miller@perkinsschools.org.
