Vaccination Clinic Planned At Perkins High School 
Before students enter Kindergarten, 7th and 12th, they are required by Ohio law to have certain immunizations.

Perkins Local Schools, in partnership with the Erie County Health Department, will host a vaccine clinic after school on April 25 at Perkins High School. Parents and guardians who want to get vaccines for their children at the clinic must schedule an appointment by calling (419) 626-5623, ext. 174.


Parents should be prepared to provide demographic and insurance information when making the appointments.


Students entering kindergarten, 7th and 12th grades are required to have certain vaccines. Records of the vaccinations – or a signed waiver indicating the vaccines were declined – are due before the start of the 2019-2020 school year in the fall, according to Ohio law.


In addition to the clinic on April 25, vaccines can be given at your children’s pediatricians’ offices or some area pharmacies. It is recommended you call ahead to ensure the vaccines are available for your child and if your insurance covers vaccinations at that location.

The Erie County Health Department also has scheduled clinics at various locations through June. A schedule of the clinics is

Copies of student vaccine records can be filed at the school offices at Furry Elementary, Briar Middle School or Perkins High School. Alternatively, the records can be faxed or email to the school nurses (see contact information below).


Required vaccinations:

  • All current 11th grade students are required to have a dose of MCV4 (Meningococcal serogroup A, C, W, and Y) before entering the 12th grade.
    The school must receive a record of the vaccine administration.
    Parents and guardians can drop off the paper records to the Perkins High School main office. The records can also be faxed to (419) 621-2057, Attention: Lisa Miller or Nurse, or emailed to nurses@perkinsschools.org


  • All current 6th grade students are required to have a booster dose of the Tdap vaccine (Tetanus, Diphtheria and acellular Pertussis) and their first dose of MCV4 (Meningococcal serogroup A, C, W, and Y) before entering the 7th grade.

The school must receive a record of the vaccine administration.

Parents and guardians can drop off the paper records to the Briar Middle School main office. The records can also be faxed to (419) 625-0523, Attention: Heather Zess or Nurse,or emailed to nurses@perkinsschools.org


  • All incoming Kindergarten students are required to submit records showing proof of all infant vaccinations and receive DTap (Tetanus, Diphtheria and acellular Pertussis), IPV (Polio), MMR (Measles, Mumps, Rubella), and Varicella (Chicken Pox) before entering kindergarten.
    The school must receive a record of the vaccine administration.
    Parents and guardians can drop off the paper records to the Furry Elementary main office. The records can also be faxed to (419) 625-6211, Attention: Melissa Koch or Nurse, or emailed to nurses@perkinsschools.org



Additional optional vaccines may also be available for children of certain ages, including HPV and Meningococcal B.
